Australian Carers Guide – Aged Care Gift Card Promotion

1. The Australian Carers Guide – Aged Care Gift Card Promotion (hereafter “the Promotion”) shall commence at 9am on Monday 20 November 2023, and end at 11.59pm on Sunday 31 December 2023 (the “Promotion Period”).

2. Aged Care Decisions shall provide any Client who places with an aged care provider or home care provider through Aged Care Decisions with a $300 Gift (the “Gift”).

3. The Gift will be provided to a Gift Card Client in the following format:

a.  Firstly, for a Gift Card Client who places with Aged Care Decisions for residential aged care, as a $300.00 AUD rebate to the relevant aged care provider for use and expenditure on additional services for the aged care resident in question, to be provided or reimbursed to the aged care provider within 28 days of the client advising Aged Care Decisions of their placement into residential aged care at a facility managed by the provider in question;

b.  Alternatively, the Gift Card Client can receive the Gift Card in the format of a $300.00 AUD Prezee Gift Card to the Client, to be provided to the client within 28 days of the client advising Aged Care Decisions of their placement into residential aged care (including details of which residential aged care facility was selected by the client);

c. Secondly, for a client who places with Aged Care Decisions for contracting with a home care provider, the Gift Card will be provided in the format of a $300 Prezee Gift Card to the Client, to be provided to the Client within 28 days of the Client advising Aged Care Decisions of the which home care provider the Client has contracted within – including details on the date of the service agreement.

4. To be eligible to receive the Gift, a person must meet the following criteria (a “Gift Card Client”):

a. Become an Aged Care Decisions Client directly having been directly referred to Aged Care Decisions from an Australian Carers Guide promotion page or promotional webpage, and not otherwise be or have been in contact with Aged Care Decisions prior (being defined as (i) having submitted a web enquiry to Aged Care Decisions, (ii) having previously had contact with Aged Care Decisions, or (iii) having previously received an electronic communication or email newsletter from Aged Care Decisions);

b. Be either the direct potential aged are resident, direct home care services recipient, or an authorised representative of a potential aged care resident or home care services recipient;

c. Be admitted with a residential aged care provider on a respite or permanent basis, or enter into a services agreement with a home care package provider for a Level 1, 2, 3 or 4 Home Care Package, that provider having been nominated on an Options Report sent to the client by Aged Care Decisions in the Promotion Period, and that Client or care recipient not having had active prior contact with the provider in question in the ninety days prior to becoming a client of Aged Care Decisions;

d. The placement with a residential aged care provider, or the entering into a services agreement with a home care package provider, must occur in the Promotion Period, or by a date no later than sixty days after the conclusion of the Promotion Period.

5. Definitions:

a. “Client” in relation to the Promotion has the same meaning as “Referred Client” pursuant to clause 2(t) of the Aged Care Decisions Provider Terms that can be found at: https://agedcaredecisions.com.au/provider-terms-2023/
