How to Access Allied Health Services — Funding Options

Chronic Disease Management (CDM) Plan 

Eligible individuals can receive 5 Medicare-subsidised allied health sessions per year via a GP referral.  

Veteran Gold Card 

Eligible veterans and dependents can access free Allied Health services through this program.   

Commonwealth Home Support Programme (CHSP) 

Entry-level support including Allied Health, designed to help seniors live safely at home. Contribution expected if affordable.   

Home Care Packages (HCP) 

For more complex care needs, funding can be allocated towards Allied Health services as part of a broader care plan. 

Support at Home (from Nov 2025) 

Replaces HCP. No out-of-pocket contributions for assessed Allied Health needs. Additional funding available for restorative care.
