in Tasmania
To apply for a Home Care Package, you need to complete an ACAT assessment through My Aged Care, and an income and assets test via Services Australia (Centrelink). You can arrange an ACAT assessment by calling My Aged Care at 1800 200 422 or by requesting a referral from your healthcare provider in Tasmania.
To be eligible for a Home Care Package, you must be 65 years or older (or 50+ if you are Aboriginal or Torres Strait Islander) and need support to live independently at home.
Funding Levels for Home Care Packages
Home Care Packages are available across four levels, and the package you receive
is determined by the assessed level of care required. Each package level provides a
specific annual budget and an estimated number of weekly care hours.
